is the box metal or plastic?

BEST ANSWER: It is plastic. I have not installed it yet but it is quite large (it is the depth of the 2x4 height plus the depth of the plaster board). I'm hoping to put both my mini cable box and its power supply inside.. This will allow me to have a flush mount TV. Good luck, -Wayne

Is there a front cover to hide the stuff inside the box?

BEST ANSWER: No cover. You might be able to fabricate something. I used it behind my wall mounted TV, so I did not need to have a cover. It worked great for my application.

what is the depth requirement? Does it fit in 2x3 stud cavity?

BEST ANSWER: No, absolutely not. This requires 3.5" depth between drywall - standard 2x4 construction.
For thinner walls you'll have to buy or create an extension ring to fit around the opening of the hole and onto which you'll mount the outside frame/flange.

Is the receptacle replaceable?

BEST ANSWER: Standard part, so yes. It just mounts in the junction box like any other junction box. The junction box is also removable.

Will this box allow for me to plug in a fairly large AC Adapter into the plug and still mount something completely flush to the wall? AC Adapter is 2.1/4”x3.1/2”, so I would require 2.1/4” inside depth.

Easy Peasy
Great box. Instructions worked for me. Make sure you have enough space left and right (avoid studs). By luck I did, but my outlet box was touching the stud.

Easy Peasy
Pretty standard 'old-work' style installation into drywall.
I'd prefer the thick brush style low-voltage access instead of the rubber with holes, but it's not enough to dock any rating points.
I also replaced the receptacle with a low clamping voltage receptacle that was 2.5x the cost of this item, but again, I didn't purchase this with the expectation that it included a high end surge suppression or line conditioning, so no rating deduction.
This box has more than enough room to handle quite a few low-voltage connections to your TV, and overall I'm quite pleased with it, especially since it is completely hidden behind my TV and since it is recessed the TV is almost flush to the wall.

Installation Nightmare
I used the template, but it simply would not fit in the hole. I made the hole slightly larger and now it has a (small) bit of it showing around the trim. Very unhappy with my first monoprice order.
