I plugged two of these into a brand new pair of M-Audio BX5as, a $400 pair of monitors. Then I unplugged them, and to my shock and horror, the tip of this adapter was lodged inside the jack. I ended up having to desolder the headphone jack to get it out. Both the speakers and the adapters were brand new. Both of the adapters I purchased displayed this problem although, luckily, only one got completely stuck.
These are constructed very cheaply, and given this experience I would strongly recommend against using them in any jacks you don't want completely ruined.
It sounds like you have received a defective product. I would be happy to assist you in getting a replacement.

Monoprice is fantastic, not flawless. Skip this one.
I'm a /huge/ advocate of MonoPrice. Their XLR cabling is just fantastic, for one! But, that said, no one is perfect. These adapters, though convenient and useful, I'm sorry to say, should be purchased elsewhere. They do not have the same quality of MonoPrice unfortunately. At least not the batch I received. Unlike MP's XLR cabling, these do not appear to be very sturdy and, in fact, I've already had one break inside a piece of equipment.
I have other adapters of this kind (I think from RatShack) and they are much sturdier. I've also had them way longer - I've only had MP's adapters for a month or so now with already one failure.
So, at least for now, I'd say pass on these. I primarily was using these as part of stereo to L/R mono splitter, which could be solved via insert cabling. So here's hoping MonoPrice comes out with some of those soon!
PROS: It's gold plated and low profile.
CONS: At least of the 4 I ordered, only 1 seems to be reliable enough to trust. The tip of one broke inside a XLR/TRS combo connector and I haven't figured out how to get it out. Likewise, at least one of the other adapters I have shows signs of failure - the collar and sleeve both rotate which doesn't seem like a good thing.
I have owned 10 of these for a number of months and they are used with a patchbay I own. So far, none of the 10 I received have broke, or become nonfunctioning. They are great to have around with a media system, and one that needs to change on the fly with a patchbay system.
While the adapter works great I would really like to know why Monoprice doesn't offer a cable that is a 6.35mm (1/4 Inch) Mono Plug to RCA. I'd much rather use a single cable as opposed to a cable and adapter.
